First and foremost, we want to thank you all for choosing Assumption Early Childhood Learning Center as your family’s early learning provider for the 2020-2021 school year!  We are so excited to get to know your child(ren) and family over the next year.

If your family is no longer interested in joining Assumption ECLC, and would like to decline the acceptance offer, please complete this form.

Please read the instructions very carefully!

Prior to starting your enrollment process, create a FACTS:

  • FACTS Accounts:
    • You MUST create a new FACTS Term:
    • Click here if you have an existing account
    • Click here if you DO NOT have an existing account
    • Please select “2020-2021 ECLC (Infant – PK4).”
    • If you have a child at ACS, they will have a separate FACTS Term.
    • ALL annual fees will be paid through FACTS. We will not be accepting cash or checks for any portion of the annual fee.
      • During the application process you will be able to choose if you want to pay 100% of your annual fee ($400), or 50% off your annual fee ($200) at time of enrollment.
      • The remaining balance may be rolled into your FACTS account and spread throughout the year.
      • The annual fee is NON-REFUNDABLE.

Printing Out the Application

  • If you PRINT OUT the application:
    • Please complete the application in LEGIBLE
    • You may DROP OFF the application at the ECLC front office. Our hours of operation are 7:00AM to 3:30PM, until further notice.
    • You may MAIL the application to:
      • Assumption ECLC

  • You may EMAIL the application:
    • Please scan pages 1 – 4 into ONE document
    • Save the application: LASTNAME_Child’sfirstname
      • Example: SMITH_Jane
    • Please email your application to: ECLC_enroll@assumptionjax.org
    • In the subject line please put: LASTNAME enrollment application
      • Example: SMITH enrollment application
      • Please include ALL children’s application in ONE email
    • Please DO NOT send any other correspondence to this email address. If you want to check your family’s enrollment status, you may call, or email me at kspears@assumptionjax.org.
    • Please DO NOT send or attach ANY medical forms, birth certificates or social security cards at this time.

Online Application:

  • If you complete the application ONLINE through Adobe:
    • You will be asked to certify and sign on each page. When you certify and sign, it will LOCK the page, and require you to save.  Please CAREFULLY review each page PRIOR to certifying and signing.
    • Page 4 will be your final signature. Save the application as LASTNAME_Child’sfirstname
      • Example: SMITH_Jane
    • Please email your application to: ECLC_enroll@assumptionjax.org
    • In the subject line please put: LASTNAME enrollment application
      • Example: SMITH enrollment application
      • Please include ALL children’s application in ONE email

Please complete ONE FORM per family once you have updated your FACTS Term, completed and submitted the enrollment application.
